Submit any one of the following:

  • 6-month bank statement (average balance > ₹10,000)
  • Latest salary slip (gross monthly income > ₹15,000)
  • ITR acknowledgement (gross annual income > ₹1,20,000)
  • Form 16 (gross annual income > ₹1,20,000)
  • Net worth certificate (> ₹10 lakh)
  • Demat holding statement (value > ₹10,000)

Senior Citizen Exception

If the applicant is unable to sign due to age or medical condition:

  • Medical certificate
  • Thumb impression on plain white paper
  • Submit these along with the full documentation
